American Traveler is hiring a Home Health RN Float requiring active MA RN licensure and prior Certified Home Health Care case management experience.

Details

• Home health float position covering the North Region including Beverly, Marblehead, Salem, and Swampstott
• 13-week assignment with scheduled shifts from 08:00-16:30
• Travel to patient homes as assigned for case management and care delivery
• Weekend rotation required approximately once every 6 weeks
• Uses Epic for electronic charting and documentation

Requirements

• Active MA RN license required, no pending or conditional licenses accepted
• Minimum 1 year of experience specifically within a certified home health agency required
• Minimum 1 year of Case Management experience required
• Experience in hospice, private care, or standalone case management roles does not meet qualification requirements
• Flu vaccine required with no religious or medical exemptions allowed
• Primary source license verification required via NURSYS and BON within 30 days of start

Additional Information

• Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing patient and family education
• Perform admission processes and case management duties per organizational policies and standards
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Local candidates may be considered if residing within 60 miles of Massachusetts General Hospital in Boston
• Uses UKG/Kronos for timekeeping
• Start date contingent on credentialing clearance, with rescheduling possible if not cleared by posted start date
