American Traveler is hiring an RN for a Home Health position requiring 1 year of home health experience and mandatory Homecare Homebase (HCHB) EMR experience.

Details

• Home health nursing services provided in patients' homes across Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas
• 8-hour day shifts, 0800-1700, 40 hours per week
• Requires completion of 6 patient visits per day, including routine visits, discharges, ROC, recerts, and reassessments
• Case management responsibilities covering 25-30 patients at once
• Average daily driving of 60-80 miles with no maximum mileage limit; mileage reimbursed weekly
• Weekend rotation not required unless interested
• On-call not required but may be available for interested candidates
• 13-week schedule cycle

Requirements

• Active RN license (NC or Compact)
• BLS certification required
• Minimum 1 year of Home Health experience required
• Homecare Homebase (HCHB) EMR experience required with no exceptions
• Valid Driver's License and proof of insurance required
• Skilled in wound care, IV therapy, pleurx, wound vac, disease teaching, and OASIS documentation
• Must reside at least 50 miles outside of the home unit
• Last 4 digits of SSN and full DOB required for consideration
• Minimum 1 professional reference dated within the last year from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Schedules patient visits one day in advance
• Cancelled appointments do not count as visit equivalents; 30 visit equivalents required weekly
• Company phone provided for documentation and communication with patients and providers; no company car provided
• Uses EPIC Rover (view only), Focura for uploading discharge instructions and wound photos, and HCHB as primary EMR
• Supported by departmental leadership, scheduler, clinical manager, clinical supervisor, and clinical peer team
• Orientation includes NEO and 5 days of unit-specific training
• Holiday coverage required for 75% of holidays
• RTO subject to manager approval
