American Traveler is hiring a Home Health RN Float requiring 1 year of certified home health agency experience and 1 year of case management experience.

Details

• Home Health setting serving the North Region, including Chelsea, East Boston, Revere, and Winthrop
• Shift schedule 08:00-16:30
• Weekend rotation once every 6 weeks
• Requires travel to patient homes for assigned cases
• Uses Epic for charting

Requirements

• Active MA RN license required, no conditional or pending licenses accepted
• BLS certification (AHA only)
• Minimum 1 year of experience within a certified home health agency required
• Minimum 1 year of case management experience required
• Hospice, private care, or standalone case management experience does not meet qualification requirements
• Flu vaccine required with no religious or medical exemptions accepted

Additional Information

• Assess patient conditions, implement care plans, and provide education and support to patients and families
• Perform admission processes and case management duties per organizational policy and standards of practice
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Local candidates accepted if permanent residence is within 60 miles of Massachusetts General Hospital in Boston, at a reduced rate
• No validated parking available at Boston locations; public transportation or paid public parking required
