American Traveler is hiring an RN Case Manager with inpatient case management experience for a Day Shift position requiring a CA RN license and case management certification.

Details

• Care Management department within RevCycle
• Day Shift, 8:00 AM-4:30 PM, Tuesday-Saturday, 40 hours per week
• No on-call responsibilities
• Heavy caseload requiring efficient management of complex patient populations
• Focus on complex patients, frequent ED utilizers, chronic pain, substance abuse, and homeless patients
• Coordinates continuum of care from pre-admission through post-discharge

Requirements

• Active CA RN license required
• BLS certification required
• RN Case Management certification required
• Inpatient case management experience required
• Experience with strike force staffing models for KP or HCA CA facilities preferred
• BSN preferred
• Minimum 2 years of experience in specialty required, with recent experience in the specialty within the last 3 months
• Must document employment history including any EMR system experience (Cerner/Epic)
• Two supervisor references required, one from the past year and one from the past two years
• Skills checklist specific to the position completed within the past year required

Additional Information

• Leads coordination of patient care across disciplines, monitoring appropriateness and timeliness of care
• Ensures interdisciplinary care plans align with clinical course and continuing care needs
• Discusses resource utilization, treatment plans, and discharge planning with physicians
• Collaborates with acute care case managers to ensure continuity of care
• Coordinates patient transfers to tertiary centers including transfer of clinical information
• Provides oversight and data collection for regulatory and accreditation compliance
• Conducts focus study reviews for frequent emergency department visitors
• Facility is Magnet-designated with a strong focus on nurse development and empowerment
