American Traveler is hiring an experienced RN Case Manager for a day shift acute care position requiring at least 1 year of case management experience.

Details

• Case Management/Discharge Planning department in a hospital setting
• Coordinates case management from patient admission through post-discharge, including outpatient settings
• Monitors patient care progress and addresses patient/family needs to resolve obstacles to effective care
• Coordinates care with payers, vendors, patients, and families
• Establishes, monitors, and executes discharge plans in collaboration with the care team
• May access and administer medications within scope of practice per state law
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM

Requirements

• Active RN license (Utah or compact) required
• BLS certification required
• Minimum 1 year of case management experience in an acute care setting required
