American Traveler is hiring a Stroke Nurse Navigator RN within a Certified Comprehensive Stroke Center, requiring an active CA RN license and 5 years of recent acute stroke or nurse navigator experience.

Details

• Serves as Stroke Nurse Navigator within a Certified Comprehensive Stroke Center
• Coordinates care for stroke patients across the continuum, from acute hospitalization through rehabilitation and outpatient follow-up
• Schedule is Monday-Friday, 8:00 AM to 4:30 PM, with occasional weekend community events
• Epic charting system experience preferred

Requirements

• Active CA RN license required
• Current certifications: NIH Stroke Scale (NIHSS) and BLS
• CNRN or SCRN certification preferred
• 5 years of recent acute care experience with stroke patients or as a nurse navigator assisting with post-discharge barriers
• Comfortable with chart review to identify gaps in stroke care and collaborate with primary nurses to close the loop
• Strong verbal and written communication skills, especially related to stroke education for staff and patients
• Knowledge of medical terminology required
• Microsoft Office experience required, including Excel and Word
• Knowledge of third-party reimbursement helpful

Additional Information

• Provides individualized education, guidance, and support to stroke patients, families, and caregivers
• Reinforces stroke prevention strategies, risk factor modification, medication adherence, and recognition of stroke warning signs
• Leads and participates in community outreach initiatives focused on stroke awareness, prevention, and education
• Participates in development and monitoring of stroke program initiatives, quality improvement activities, and regulatory compliance to maintain Comprehensive Stroke Center certification
• Collaborates with inpatient, outpatient, rehabilitation, and community partners to improve transitions of care
• Must reside within a 50-mile radius to qualify for local candidacy
• Accepts local candidates at the local rate
• Returning staff or travelers must have been separated from the facility for at least 1 year before returning
• Maximum RTO of 7 days per contract, with only 1 holiday able to be requested per contract
• Recognized holidays include New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
