American Traveler is hiring an experienced RN Case Manager for a PACE program setting, requiring home health case management experience and a CA RN license.

Details

• Home health case management setting within a PACE program
• Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
• Manages an assigned caseload with a designated visit productivity standard
• Coordinates interdisciplinary team to implement patient care plans
• Oversees visit utilization management and expense control
• May assist with agency-wide orientation and competency evaluations
• Epic charting system preferred

Requirements

• Active CA RN license required
• BLS certification required (AHA or ARC)
• Minimum 2 years of RN experience required
• Home health case management experience required
• Prior charge experience required
• Skills in care coordination, discharge planning, and disease management preferred
• 2 supervisor references obtained within the past year required

Additional Information

• Responsible for overall management of patients including direct care delivery
• Ensures optimal quality of care in accordance with current standards and regulations
• May provide consultation to clinical staff related to specific areas of expertise
• Local candidates accepted at local rate within a 50-mile radius
• Returning staff must have a 1-year separation before returning as a traveler
• Maximum of 7 days RTO per contract, with only 1 holiday request approved per contract
• Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
• Must be available for lab work within 3-5 days of contract signing
• First-time travelers are not being considered for this position
