American Traveler is seeking an RN - Home Health with at least 1 year of home health nursing experience and required Homecare Homebase EMR experience for a home visit position across multiple cities.

• Home health setting making visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and surrounding areas
• No maximum mileage; average daily driving is 60-80 miles, with mileage reimbursed weekly
• Central housing near University City and/or Concord recommended, with willingness to float throughout the coverage area
• 8-hour day shift schedule, 40 hours per week, with start times between 7-9 AM
• Required to complete 6 patient visits per day, including routine visits, discharges, ROC, recertifications, and reassessments
• Required to meet 30 visit equivalents per week
• Case management responsibilities overseeing 25-30 patients at a time
• Patient population includes chronic diseases, wound care/wound vac, IV therapy, and pleurx management
• No weekend rotation required unless volunteering or applying for a weekend-specific role
• Call is not required but may be available for interested candidates
• Company phone provided for documentation and patient/provider communication
• No company car provided
• Clinic days vary by branch, typically one in-person day per week with some virtual options

Requirements

• Active NC or compact RN license required
• BLS certification required
• Minimum 1 year of home health nursing experience required, with most recent experience within the last year
• Valid driver's license and proof of insurance required
• Homecare Homebase EMR charting experience required with no exceptions
• EPIC Rover experience (view only) used for documentation
• Focura experience used for uploading discharge instructions and wound documentation
• Skills required include IV therapy, pleurx management, wound vac care, wound care, patient assessment, disease teaching, OASIS, and case management
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum 1 professional reference dated within the last year from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Schedule patient visits one day in advance
• Support provided by departmental leadership, scheduler, clinical manager, clinical supervisor, and clinical team
• Orientation includes NEO and 5 days with a preceptor
• Expected to work all holidays unless manager approval is obtained for time off
• RTO requires manager approval
• 13-week schedule cycle

• Must reside more than 50 miles from the facility to be eligible for this position
• Returning employees must have been separated from any AHSE facility for at least 1 year after working a permanent position
