American Traveler is seeking an RN Case Manager with at least 4 years of RN experience and required Epic charting experience for a day-shift acute hospital role.

Details

• Acute hospital setting within a Case Management department
• Focus on case management, utilization review, and discharge planning
• Average daily census of approximately 70 on weekends and 20 on weekdays
• Day shift schedule, Monday through Friday, 8:30 AM to 5:00 PM, 5x8 hours
• No on-call responsibilities
• 24 hours of orientation provided

Requirements

• Active RN license in North Carolina (or compact)
• BLS certification required; ACM or CCM certification preferred
• Minimum 4 years of RN experience required
• Minimum 2 years of healthcare experience as an RN
• Recent acute hospital experience required
• Epic charting system experience required
• Graduate of an accredited nursing program; BSN preferred
• First-time travelers will not be considered

• Provide comprehensive assessment, planning, implementation, and evaluation of individual patient care needs
• Support continuity of care and cost-effective outcomes through integration of case management, utilization review, and discharge planning functions
• Must be highly organized, detail-oriented, and adaptable to frequent change while maintaining compliance with regulatory and departmental guidelines
• RTO allowed except during Nov 25-27, Dec 24-25, and Dec 31-Jan 1
• Must reside outside a 50-mile radius of the facility; no local candidates accepted
• Former employees of the health system must be disclosed if applicable
