American Traveler is hiring an RN Case Manager for a Home Health position requiring 2 years of home health experience and a VT/NH or compact RN license.

Details

• Home health case management across VT and NH, covering areas including Bradford, Randolph, Springfield, White River Junction, and Lebanon
• Serves a home-based patient population requiring nursing, rehabilitation, hospice, and personal care services
• Full-time schedule, days
• 16-week assignment

Requirements

• Active RN license in VT and NH or unencumbered compact license
• BLS/BCLS certification required
• Graduate of an accredited school of nursing
• Minimum 2 years of experience in home health

Additional Information

• Develop and periodically review individualized care plans in collaboration with physicians, patients, and families
• Coordinate care through interdisciplinary communication and case conferences, including timely discharge notifications
• Provide ongoing clinical, functional, and emotional assessments and communicate significant changes to physicians and colleagues
• Document evaluations, treatment goals, and objectives
• Teach patients and caregivers self-care techniques and provide medication, diet, and health counseling as ordered
• Work independently while remaining part of a cohesive interdisciplinary care team
