Description

American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring at least 1 year of certified home health and case management experience along with an active MA RN license.

Details

• Home health setting covering the North Region including Beverly, Marblehead, Salem, and Swampscott
• Float role requiring travel to patient homes as assigned
• Day shift schedule from 8:00 AM to 4:30 PM
• Weekend rotation required once every 6 weeks
• Uses Epic for charting

Requirements

• Active MA RN license required (no conditional or pending licenses accepted)
• Both NURSYS and Board of Nursing verification required for licensure
• AHA-issued certifications only
• Minimum 1 year of experience specifically within a certified home health agency required
• Minimum 1 year of case management experience required
• Hospice, private care, or standalone case management experience does not meet qualification requirements
• Flu vaccine required with no medical or religious exemptions

Additional Information

• Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
• Perform admission processes and case management responsibilities per organizational policy and standards
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Local candidates must reside within 60 miles of Massachusetts General Hospital in Boston
• Start date contingent on credentialing clearance, with rescheduling possible if not cleared by the posted start date
