American Traveler is hiring a home Hospice RN requiring two years of home hospice experience and a NJ or compact RN license.

Details

• Home-based hospice and palliative care setting
• Schedule includes 5 days per week, 7.5-hour shifts from 8a-4p
• Requires working every other weekend with comp days during the week
• Float required to different counties as needed based on agency demand
• No on-call responsibilities
• Uses Epic for EMR charting

Requirements

• Active NJ or compact RN license required
• BLS certification required
• Valid New Jersey driver's license required
• Minimum 2 years of RN experience in a home hospice environment required
• Associate's (ASN) degree or diploma with RN license in New Jersey required
• Experience using an EMR required
• Strong critical thinking and communication skills required

Additional Information

• Performs patient assessments to determine hospice appropriateness, learning needs, and progress toward care goals
• Develops, implements, and revises individualized plans of care in collaboration with the interdisciplinary team (IDT)
• Completes admission, discharge, and re-visit documentation within required timeframes
• Uses age-appropriate teaching methods and evaluates effectiveness of patient/family education
• Maintains safe care environment through infection control and safe medication management practices
• Attends IDT meetings biweekly and monthly staff meetings
• Participates in performance improvement (PI) activities and may provide mentorship to new employees
• May float to a nearby sister hospital as needed
