American Traveler is hiring an experienced RN Case Manager for an acute hospital setting, requiring 5+ years of RN case management experience and inpatient acute case management background.

• Acute hospital Case Management department
• Day shift schedule, 5x8 hours, 8:00 AM-4:30 PM
• Every other weekend required
• 1:10 patient ratio

Requirements

• Active CA RN license required
• BLS certification required
• 5+ years of RN Case Management experience required
• Inpatient Acute Case Management (hospital) experience required
• Discharge planning experience required
• 2 supervisor references obtained within the past year required

Additional Information

• Responsible for care coordination and care transitions planning throughout the acute care patient experience
• Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression and transition of patients to appropriate levels of care
• Conducts patient assessments to determine readmission risk and screens for ancillary supportive services including Palliative Care
• Reviews medical records to ensure patients meet level of care requirements and documentation supports LOC determination
• Monitors length of stay and collaborates with financial counselors on inpatient stay denials
• Formulates transition plans in collaboration with patients, families, physicians, and community-based support services
• Screens 30-day readmissions and creates effective transition plans with the interdisciplinary team
• Identifies ED high utilizers and makes appropriate care plans and community resource referrals
• Maintains current knowledge of CMS, Joint Commission Transitions of Care requirements, and Conditions of Participation
• Candidates must reside within a 50-mile radius to qualify for local rate
• Returning staff/travelers must have a 1-year separation before returning as a traveler
• Maximum RTO is 7 days per contract, with only 1 holiday request approved per contract
• Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
• Must be available for lab work within 3-5 days of contract signing
