American Traveler is hiring a Case Management RN for a Days schedule requiring at least 2 years of case management experience and 6 months of acute care discharge planning experience.

Details

• Acute care hospital case management unit
• Day shift, Monday through Friday, 07:30-18:00
• 40 hours per week, potentially structured as 4x10s or 5x8s depending on operational need
• 13-week assignment length
• Uses Epic for charting

Requirements

• Active MA RN license required; pending or conditional licenses not accepted
• Minimum 2 years of case management experience required
• Minimum 6 months of discharge planning experience in an acute care hospital required
• Experience in a large teaching hospital with 500+ beds strongly preferred
• AHA-issued certifications required
• Primary source verification of license/certifications required within 30 days of start, updated upon expiration
• Both NURSYS and Board of Nursing verification required for nursing license

Additional Information

• Responsible for discharge planning and coordination within the case management department
• Flu vaccination required with no religious or medical exemptions allowed
• Two professional references required at time of consideration
• RTO must be disclosed at time of consideration
• Local candidates must reside within 60 miles of the facility location to be eligible for the local rate
• Uses UKG/Kronos for timekeeping
