American Traveler is seeking an experienced RN Hospice Case Manager for a Home Health/Hospice position requiring 2 years of hospice experience and an active NJ or Compact RN license.

Details

• Home Health/Hospice case management setting
• Day shift schedule, 5x8-hour shifts from 8:30 AM to 5:00 PM
• Travel required to patients across Burlington, Camden, and Gloucester counties
• Patient ratio of 1:5
• Epic EMR experience required

Requirements

• Active NJ or Compact RN license
• Current BLS certification that does not expire within 30 days of assignment start
• Minimum of 2 years Home Care Hospice experience within the last 12 months
• Epic charting system experience required
• Proficiency in pain management including WHO Analgesic Ladder, breakthrough dosing, and PCA pump management
• Skilled in symptom assessment and management including altered mental status, cachexia, and pressure ulcer staging
• Experience with end-of-life care including bereavement planning, death pronouncement, and postmortem care
• Knowledge of Medicare state regulations and DME authorization processes
• Familiarity with ethical and legal issues such as palliative sedation and withholding/withdrawing treatment

Additional Information

• Serve as Hospice Case Manager providing patient and family education and support
• Conduct intake assessments and goal setting with patients and families
• Provide culturally aware end-of-life care and perideath support to families
• Navy blue scrubs required
• First-time travelers will be considered if candidate has hospice experience
• COVID-19 and Flu vaccinations required with no exemptions
• Candidates residing within 50 miles of the facility are subject to the local pay rate
• RTO requests must be submitted with the initial profile and no RTO is permitted during the first week of the assignment
