Description

American Traveler is seeking a Hospice RN with a UT or compact license and at least 3 years of nursing experience, preferably in hospice, home health, or palliative care.

Details

• Hospice care setting providing services to terminally ill patients in their preferred place of care, typically the home
• Day shift schedule over a 13-week assignment
• On-call rotation required, 1-3 shifts per week including 3 weekends during the contract depending on staffing levels
• Must be able to respond within 45 minutes to patient needs when on call
• Hospicesoft/Careficient EMR experience preferred

Requirements

• Active RN license in Utah or compact state
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care
• Current BLS certification required
• Strong clinical assessment skills
• Valid driver's license and reliable transportation required for home visits
• Successful completion of a pre-employment drug test and background check required

Additional Information

• Perform comprehensive assessments of patients' needs and coordinate individualized care plans
• Provide pain and symptom management consistent with the hospice plan of care
• Educate and support patients and families regarding disease process, self-care techniques, end-of-life issues, and hospice philosophy
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and volunteers
• Maintain accurate and timely documentation in compliance with state, federal, and organizational policies
• Must be able to work holidays
