American Traveler is hiring a Home Health RN in Mesa County, CO, requiring an active CO or compact RN license and offering a weekday schedule with on-call rotation.

Details

• In-person home health position providing skilled, patient-centered care in the community
• Monday-Friday day schedule with a flexible day-to-day workflow
• Includes participation in an on-call rotation
• Manageable caseloads with one-on-one patient care
• EMR documentation required, with HCHB experience preferred
• Coordinates care with LPNs, therapists, caregivers, and providers

Requirements

• Active RN license in Colorado or active NLC compact license
• Current BLS/BCLS certification
• Valid driver's license, reliable transportation, and auto insurance required
• Strong assessment and critical thinking skills
• Comfort with documentation and EMR systems, HCHB experience preferred
• Home health experience preferred but not required
• Ability to work independently and manage a structured caseload

Additional Information

• Perform comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care and appropriate clinical interventions
• Monitor patient progress and communicate changes to the care team
• Educate patients and families on medications, treatments, and care plans
• Complete timely and accurate documentation in the EMR system
• Work within a supportive, interdisciplinary team
• Independent visits conducted in patients' homes
