American Traveler is hiring an experienced Hospice RN for a home hospice and palliative care community position requiring NJ or compact RN licensure and two years of hospice nursing experience.

Details

• Home hospice and palliative care community setting
• Day shift schedule from 8:00 AM to 4:00 PM
• 37.5 hours per week, 7.5 hours per day, 5 days per week
• Every other weekend required, with comp days provided during the week
• One summer and one winter holiday coverage required
• No on-call responsibilities
• Floating as needed, including to a sister hospital nearby
• Uses Epic EMR for charting

Requirements

• Active NJ RN license (Standard or Compact) required
• BLS certification required
• Valid New Jersey driver's license required
• Minimum two years of experience as an RN in a home hospice environment required
• Three or more years of RN experience preferred
• Prior experience working within a hospice agency/organization preferred
• Associate's (ASN) degree or Diploma with NJ RN license required
• EMR charting experience required
• Strong critical thinking and communication skills required

Additional Information

• Conduct patient assessments to determine hospice appropriateness, learning needs, and progress toward goals
• Develop and revise individualized plans of care for patients and families
• Complete admission assessments and data transfers within 24 hours, plus twice-daily laptop data transfers
• Provide age-appropriate patient and family education and document tools used
• Manage caseload including prioritizing care needs, scheduling, and interdisciplinary communication
• Perform HHA supervisions per standards and ensure appropriate utilization of services
• Attend and participate in interdisciplinary team (IDT) meetings every two weeks
• Maintain safe environment through infection control and safe medication management practices
• Reconcile medications routinely and maintain current medication orders
• Attend monthly staff meetings and participate in performance improvement (PI) activities
• Provide orientation and mentorship to new employees as needed
• Local candidates accepted
• Returning travelers may stay on assignment up to one calendar year from initial start date before requiring a break
