American Traveler is hiring a Registered Nurse for a Home Hospice position requiring two years of hospice nursing experience and a valid NJ driver's license for travel between patient locations.

Details

• Home hospice and palliative care setting covering Morris & Sussex Counties
• PM shift from 4:00pm to 8:00am, 3 shifts per week, 37.5 hours weekly
• Weekend rotation required every 3 weeks with comp days provided
• One summer or winter holiday coverage required
• Floating as needed to sister facility
• No on-call responsibilities
• Uses Epic for charting

Requirements

• Active NJ RN license (Standard or Compact) required
• Valid NJ driver's license required
• BLS certification required
• Minimum 2 years of RN experience in a home hospice environment required
• Associate's (ASN) degree or diploma with NJ RN license required
• Prior EMR charting experience required
• Excellent critical thinking and communication skills required

Additional Information

• Perform comprehensive patient assessments to determine hospice appropriateness, learning needs, and progress toward goals
• Develop and revise individualized plans of care for patients and families
• Complete admission assessments and data transfers within 24 hours, along with twice-daily laptop data transfers
• Provide age-appropriate patient and family education and document teaching tools used
• Manage caseload including HHA supervision, medication reconciliation, and safe medication management
• Participate in interdisciplinary team (IDT) meetings every 2 weeks and communicate patient progress
• Attend monthly staff meetings and participate in performance improvement (PI) activities
• Provide orientation and mentorship to new employees
• Maintain a safe environment through infection control practices
