American Traveler is hiring a Registered Nurse for a Home Hospice position requiring 2+ years of home hospice RN experience and a current NJ RN license.

Details

• Home care hospice and palliative care setting
• Provides on-call coverage for Morris and Sussex Counties
• Schedule includes 3 shifts per week, 16 hours per shift (4pm to 8am)
• Weekend rotation required every 3 weeks with comp days provided
• One summer or winter holiday coverage required
• Floating may be required to a nearby sister hospital as needed
• Uses Epic EMR for charting

Requirements

• Active NJ RN license or compact license
• BLS certification required
• Valid New Jersey driver's license required
• Minimum 2 years of RN experience in a home hospice environment required
• 3+ years of RN experience preferred
• Experience within a hospice agency or organization preferred
• Associate's degree (ASN) or Diploma with NJ RN license required
• EMR charting experience required
• Strong critical thinking and communication skills required

Additional Information

• Performs patient assessments to determine appropriateness for hospice care and develops individualized plans of care
• Completes admission, discharge, and re-visit documentation with data transfers via laptop
• Provides patient and family education using age-appropriate teaching methods
• Manages caseload including care plan development, revision, and prioritization based on patient needs
• Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines
• Communicates patient progress with the interdisciplinary team (IDT) and attends IDT meetings biweekly
• Supervises home health aides (HHA) per required standards
• Follows infection control protocols and maintains a safe environment for patients and staff
• Manages medications including reconciliation and maintaining current orders
• Attends monthly staff meetings and participates in performance improvement (PI) activities
• Provides orientation and mentorship to new employees and cross-trains on new skills
