Description

American Traveler is hiring an RN Case Manager for Discharge Planning with at least 2 years of case management experience and 6 months of acute care discharge planning experience.

Details

• Acute care hospital setting within the Case Management department
• Day shift, Monday through Friday, 07:30-18:00
• 40 hours per week, with schedule potentially structured as 4x10s or 5x8s depending on operational need
• 13-week assignment length
• Uses Epic for charting

Requirements

• Active MA RN license required, no conditional or pending licenses accepted
• Minimum 2 years of case management experience required
• Minimum 6 months of discharge planning case management experience in an acute care hospital required
• Experience in a large teaching hospital with 500+ beds strongly preferred
• AHA certifications only
• Flu vaccine required with no religious or medical exemptions allowed

• Responsible for discharge planning and case management duties within an acute care hospital
• Local candidates must reside within 60 miles of the facility
