American Traveler is hiring an RN Case Manager with required utilization review experience for a hospital-based case management position with day shift and weekend rotation requirements.

• Hospital-based Case Management role supporting Med-Surg Adult patients
• Day shift schedule from 7:30am to 4:30pm with weekend rotations
• Collaborates with physicians and multidisciplinary teams to develop patient care plans
• Uses EMR, MS Office, and standard office equipment

Requirements

• Active Montana RN license (or Compact) required
• Current BLS certification (American Heart Association) required
• Minimum 2 years of nursing experience required
• Utilization review experience required
• Bachelor's Degree in Nursing preferred
• 2 years of acute care hospital case management experience preferred
• Strong interpersonal, verbal, and written communication skills required
• Ability to lift up to 50 pounds repeatedly and perform frequent standing, walking, bending, and reaching

Additional Information

• Conducts comprehensive patient assessments to determine case management needs based on clinical judgment and identified triggers
• Develops and implements individualized case management plans in collaboration with patients, caregivers, and healthcare team members
• Monitors and documents care plan effectiveness, adjusting interventions as needed to achieve desired outcomes
• Maintains an ongoing patient caseload requiring regular outreach and coordination
• Facilitates integration of behavioral health and community support services to enhance continuity of care
• Actively participates in discharge planning to identify and resolve barriers to effective patient care
• May require on-call availability and occasional work beyond standard 40-hour week, including weekends
• Facility may cancel up to three shifts per 13-week contract due to low census or unforeseen circumstances
• Candidates must reside at least 90 miles from the facility to be considered
• Fast-paced environment requiring sound judgment, professionalism, and strong emotional intelligence
