American Traveler is hiring an experienced Hospice RN Case Manager for a home care assignment requiring at least 1 year of hospice or acute care experience and a MO or compact RN license.

Details

• Home care/hospice setting supporting patients and families at end of life
• RN Case Manager role covering a caseload of 15-18 patients with approximately 5 visits per day
• Duties include visit nursing, medication and supply management, DME management, wound care, and patient/family teaching
• Responsibilities include admissions, triage visits, and routine visits
• Handles high acuity care including port access, wound vac, pleurX drains, peritoneal drains, and trach care
• Familiarity with CADD pain pumps is a plus
• Schedule is Monday-Friday, 8:00am-4:30pm
• Required to work every other weekend
• Territory coverage spans a 40-80 mile radius
• Uses Epic for charting
• Travelers will float within the service line as needed

Requirements

• Active MO or compact RN license required
• BLS certification required
• Minimum 1 year of hospice experience in home health or inpatient setting, or 1-2 years of acute care experience required
• Pediatric experience preferred
• Strong computer skills required
• Experience with vents preferred
• Epic charting experience required

Additional Information

• Business casual attire with closed-toe shoes required
• Candidates must reside at least 75 miles from the facility, as no local candidates are accepted
• RTO limited to a maximum of 7 days per 13-week contract, with no time off allowed during the first week of the assignment
• First-time travelers are welcome to apply
• Returning permanent employees must wait 1 year before returning to any BJC facility as a traveler
• Standard travelers may stay for an 18-month period and must take a 3-month break in service afterward
• Mileage reimbursement provided for travel between patient visits
