American Traveler is hiring a Home Health RN case manager for a coastal Maine assignment requiring 2+ years of home health experience and EPIC/OASIS proficiency.

Details

• Home health case management setting serving the Lincoln County/Coastal area of Maine
• Case load of 20-25 patients with an expectation of 5-6 visits per day
• May also be utilized as a re-visitor based on agency needs
• Day shift, 8:00 AM - 5:00 PM
• Permanent weekly schedule options may include Monday-Friday, Tuesday-Saturday, or Sunday-Thursday depending on agency needs
• Every other weekend rotation required with a day off during the week
• Call back required 2-4 times per month between 5:00 PM and 8:00 PM, with no overnight call
• No floating required, though a 25-mile float policy between facilities may apply as needed
• Adult patient population including med surg, oncology, cardiac, neurological, respiratory, wound care, GI, and GU cases
• Uses EPIC EMR
• Assigned agency laptop, iPhone, nursing supply bin, nursing bag with vital sign equipment, and INR machine for the duration of the assignment

Requirements

• Active RN license (Maine or Compact) required
• BLS certification required
• Minimum 2 years of RN experience required
• EPIC and OASIS experience required
• Home health case management experience required
• Preferred experience with IV therapy, tube feeds, wound VAC dressings, compression wraps, port access, ostomies, chest tubes, suprapubic catheters, and lab draws
• Driver's license required for consideration
• Two professional references required

Additional Information

• Provide comprehensive home health case management including patient assessments, visits, and care coordination
• Work independently in patients' homes while maintaining strong communication with the care team
• Organized, collaborative, and detail-oriented approach to documentation and workflow required
• Business casual attire or scrubs of any color permitted; jeans, open-toed shoes, and clothing with holes are not allowed
• Candidates residing within 90 miles of any MaineHealth facility will be considered local, which may affect pay rate
• Local candidates are not eligible for contract renewals or extensions
• Recommended time off for this 13-week contract is limited to 3-4 days, subject to manager discretion
• Candidates who have worked at any MaineHealth facility within the past 12 months should disclose this during the application process
