American Traveler is hiring a travel RN for a Home Care Hospice & Palliative Care position in Morristown, NJ, requiring a minimum of 2 years of home hospice RN experience and a valid NJ driver's license.

• Home care/hospice setting serving patients in their place of residence across multiple counties
• Hospice and palliative care patient population
• AM shift, 8:00 AM – 4:00 PM, Monday through Friday
• 37.5 hours per week, 7.5 hours per day
• Every other weekend required, with compensatory days off during the week
• Floating to different counties based on patient need
• No on-call responsibilities
• EMR: Epic (experience required)

Requirements

• Active NJ RN license (NJ or Compact) currently in hand
• Current BLS certification required
• Minimum 2 years of RN experience in a home hospice environment
• Valid New Jersey driver's license required
• Associate's degree (ASN) or Diploma with RN licensure required
• Strong critical thinking and communication skills
• Experience using an EMR system
• 2 professional references required for consideration

Additional Information

• Performs patient assessments to determine appropriateness for hospice care and develops individualized plans of care for patients and families
• Manages a caseload as the primary case manager, including developing, implementing, and evaluating evidence-based care plans
• Completes laptop data transfers twice daily and admission/re-visit documentation within required timeframes
• Demonstrates knowledge of hospice regulations, insurance reimbursement guidelines, and safe medication management
• Participates in interdisciplinary team (IDT) meetings every two weeks and communicates patient progress with the team
• Provides patient and family education using age-appropriate teaching methods and documents tools utilized
• Participates in performance improvement activities, UR meetings, and provides orientation/mentorship to new staff
• Candidates must reside more than 50 miles from the facility — local candidates are not eligible
• Travelers who have been on assignment for one full calendar year must take a 30–60 day break before beginning a new assignment at this facility
