American Traveler is hiring an LPN/LVN with an active NY license for a long-term care assignment featuring night shifts in a state-run skilled nursing and rehabilitation facility.

Details

• State/government-run long-term care center offering rehabilitation therapies, skilled nursing, memory care, and wound care
• LTC unit serving a patient population that includes Alzheimer's and dementia residents
• 300-bed facility with a nurse-to-resident ratio of approximately 1:18–20
• Scheduled for night shifts (2345–0715), 8 hours per day, 5 days per week
• Night shift is described as slower paced
• Every other weekend required upon center request
• Uses Point Click Care (PCC) for charting; experience not required
• 16-week contract

Requirements

• Active NY State LPN/LVN license required; no license violations accepted — license required if applicable
• Minimum 1 year of LTC experience required; candidates with recent rehab, med-surg, or hospital experience will also be considered
• Current BLS certification required
• Must be able to pass a medication administration (med pass) test and clinical observation
• Strong attendance, punctuality, and dependability required
• 2 professional supervisory references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration
• Proof of NY State license required for consideration

Additional Information

• Responsibilities include medication administration and accurate medication passes, monitoring and reporting changes in resident condition, timely documentation, and supervising CNAs on assigned units

• All time-off requests must be submitted at the time of initial consideration; RTO will not be approved after an offer is extended
• Shift cannot be changed once an offer is extended
• Holiday time off cannot be taken for every holiday; some holiday coverage is required
• Call-outs must be provided with more than 24 hours notice
• Compliance process takes approximately 1.5–2 weeks
• Upon offer acceptance, Relias training modules will be sent via email within 48 hours and must be fully completed no later than 3 days prior to the assignment start date
• Any solid color scrubs and closed-toe shoes required
