American Traveler is hiring an experienced RN for a Hospice & Palliative Care Home Health position requiring a minimum of 2 years of home hospice RN experience and a valid NJ driver's license.

Details

• Home health/hospice setting with General Inpatient (GIP) care responsibilities
• Shift runs 11:00 AM – 7:00 PM, 5 days per week (7.5 hours/day, 37.5 hours/week)
• Every other weekend (EOW) required
• On-call responsibilities are required
• Previous ICU experience is preferred
• Charting is performed using Epic EMR
• Floating to a nearby sister facility may be required

Requirements

• Active NJ RN license (NJ or Compact) currently in hand
• Current BLS certification required
• Minimum 2 years of RN experience in a home hospice environment required
• 3+ years of RN experience preferred
• Valid New Jersey driver's license required
• Associate's degree (ASN) or Diploma with NJ RN license required
• Experience with an EMR system required; Epic experience preferred
• Strong critical thinking and communication skills required
• Two professional references required for consideration

Additional Information

• Conduct patient assessments to determine appropriateness for hospice care and develop individualized care plans for patients and families
• Manage a caseload as the primary case manager, including care plan development, revision, and interdisciplinary team (IDT) communication
• Attend and participate in IDT meetings every two weeks and monthly staff meetings
• Perform HHA supervisions, medication reconciliation, and safe medication management
• Complete admission assessments and documentation transfers within 24 hours using a laptop-based system
• Provide patient and family education using age-appropriate teaching methods and document tools used
• Participate in Performance Improvement (PI) and Utilization Review (UR) activities, and assist with orientation and mentorship of new staff
