American Traveler is seeking an experienced RN for an Outpatient Hospice position in Oregon, requiring an active OR RN license and community-based care experience.

Details

• Outpatient hospice setting serving patients in their own homes, skilled nursing facilities, assisted living facilities, and adult foster homes
• Serves patients across Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties
• Services include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aide coordination, and bereavement support
• 24-hour on-call nursing services are part of the program structure
• Variable shift schedule with rotating weekends
• 13-week contract

Requirements

• Active, unencumbered Oregon RN license currently in hand
• Current BLS (Healthcare Provider level) certification required
• Minimum 1 year of nursing experience required
• BSN and/or previous home health, hospice, or case management experience preferred
• Broad-based nursing experience to support the specialized skills required for home health and hospice nursing preferred
• Valid driver's license and personal auto liability/property damage insurance required, as this position requires use of a personal vehicle
• Must maintain personal vehicle in good working order

Additional Information

• Provides skilled nursing visits and coordinates comprehensive hospice care including pain and symptom management for patients in community-based settings
• Supports patients and families through end-of-life care, coordinating with interdisciplinary teams including social workers, spiritual care providers, hospice aides, and volunteers
• Medications and equipment are provided to patients as appropriate
