American Traveler is seeking a compassionate RN for a Hospice Care Manager position in a home health setting in Vermont, requiring OASIS experience and an active VT or compact RN license.

Details

• Home health and hospice setting, providing care in patients' homes
• Hospice and care management patient population, focused on end-of-life care
• Day shift schedule, Monday through Friday with some weekend coverage
• 16-week contract

Requirements

• Active VT or compact RN license currently in hand
• Current BLS/BCLS certification required
• Minimum 1 year of RN experience required
• Home health experience required
• OASIS documentation experience required
• Case management experience required

• Provide compassionate end-of-life care, addressing the physical and emotional needs of patients and their families
• Deliver expert pain and symptom management to hospice patients
• Collaborate with an interdisciplinary team, local physicians, and community providers to optimize patient quality of life
