American Traveler is hiring a travel RN for a Case Management position at a large academic medical center, requiring active RN licensure, current inpatient case management experience, and Epic proficiency.

Details

• Day shift position, 5x8-hour shifts per week, 8:30 AM – 5:00 PM
• No weekend, holiday, or on-call requirements
• Inpatient case management unit within an adult medicine/surgery setting on a main hospital campus
• Manages a caseload of adult patients across care coordination, transition planning, and post-acute referrals
• Uses Epic EMR and post-acute referral platforms such as NaviHealth or Allscripts

Requirements

• Active MD or Compact RN license required
• Minimum 2 years of RN experience required
• Current inpatient case management experience required
• Current BLS certification required
• Experience with Epic EMR required
• Experience with post-acute referral platforms (e.g., NaviHealth or Allscripts) required
• Experience in a facility with 500+ beds required
• SSN and DOB required for consideration
• 1 professional reference required for consideration

Additional Information

• Coordinates care for a select caseload of adult patients from triage through safe transition to home or post-acute settings
• Proactively communicates with payors to secure approval of treatment plans and required resources
• Collaborates with multidisciplinary teams, patients, and families to ensure effective transition planning
• Coordinates transitions to post-acute settings including skilled nursing facilities, acute inpatient rehab, and community resources such as local shelters
• Maintains patient information in all departmental systems per operational guidelines
• Local candidates are accepted at the same rate as travelers
