American Traveler is hiring an RN Case Manager for a short-term acute care hospital in Portland, OR, requiring 2 years of case management experience, Epic proficiency, and flexibility to float across multiple facilities.

Details

• Short-term acute care hospital setting with 418 staffed beds
• Case Management unit with a 1:20 patient ratio
• Day shift schedule, 5x8 hours, 7:00 AM – 3:00 PM
• Every other weekend required
• Floating required to other units based on skill set and facility needs; affiliate facilities include locations in Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and Saint Vincent
• Epic EMR experience required
• No on-call required
• A personal vehicle is required for this assignment

Requirements

• Active Oregon RN license required; may submit with licensure pending
• Current BLS certification required; Case Management Certification preferred
• Minimum 2 years of case management RN experience required
• Experience in an acute hospital setting required
• Proficiency with Epic EMR required
• First-time travelers are welcome to apply
• Must reside more than 50 miles from the facility to qualify for travel pay rates; local candidates may be accepted at a different rate
• 2 supervisory references covering at least 1 year of employment within the last 3 years in a comparable specialty required for consideration
• SSN and DOB required for consideration

Additional Information

• Responsibilities include discharge planning, care coordination, needs assessment, DME ordering, plan of care development, prior authorizations, and determining medical necessity per evidence-based guidelines
• Knowledge of regulatory frameworks including CMS, DRG, HIPAA, OSHA, and The Joint Commission/Core Measures expected
• Experience in behavioral health, home health, hospice, or LTAC settings is preferred
• Familiarity with appeals and denials, admission criteria, and HEDIS measures is preferred
• Scrubs of any color are acceptable for this unit
• Must commit to working 3 of the following 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
• Up to 6 days of RTO (or 3 consecutive days) may be considered; all RTO requests must be submitted upfront
• Former permanent staff must wait at least 3 months before returning as a traveler
• Candidates must disclose any prior work history at a Providence facility (permanent, travel, or per-diem), including location and dates
