American Traveler is seeking an experienced RN for a Case Management position at a large academic medical center, requiring active RN licensure and current inpatient case management experience.

Details

• Case Management unit within a large academic hospital (main campus)
• Adult Medicine/Surgery patient population
• Day shift: 8:30 AM – 5:00 PM, five 8-hour shifts per week
• No weekend, holiday, or on-call requirements
• Uses Epic EMR; experience required
• Experience with post-acute facility referral platforms such as NaviHealth or Allscripts required

Requirements

• Active RN license (Maryland or Compact) currently in hand
• Current BLS certification required
• Minimum 2 years of RN experience required
• Current inpatient case management experience required
• Experience in a large hospital setting (500+ bed facility) required
• Proficiency with Epic EMR required
• Experience with post-acute referral platforms (e.g., NaviHealth or Allscripts) required
• SSN and DOB required for consideration
• One professional reference required for consideration

Additional Information

• Coordinates care for a select caseload of adult patients from triage through transition to home or post-acute settings
• Proactively communicates with payors to ensure approval of treatment plans and required resources
• Collaborates with the multidisciplinary team, patients, and families to facilitate safe and effective transition planning
• Coordinates transitions to post-acute settings including nursing facilities, acute inpatient rehab, and/or local shelters
• Enters and maintains patient information across departmental systems per operational guidelines

• Medication administration is not part of this role; no medication competency testing required
