Description

American Traveler is seeking an experienced RN Case Manager for an inpatient care management position requiring a BSN, certified care manager credential, and recent Level 1 or comparable Level 2 Trauma experience.

Details

• Inpatient hospital setting managing a daily caseload of patients across multiple units with a variety of care needs
• Monday through Friday schedule, 8:00 AM to 4:40 PM
• No weekend or on-call requirements
• Uses Epic EMR for documentation and order placement

Requirements

• Active NH or Compact RN license required
• BSN required
• Current BLS certification required
• Certified Care Manager (CCM) credential required
• Minimum 3 years of inpatient clinical experience required
• Minimum 1 year of care management experience required
• Recent Level 1 or comparable Level 2 Trauma experience within the last 2 years required
• 2 manager references required for consideration

Additional Information

• Conduct initial assessments and attend interdisciplinary rounds
• Facilitate individualized plans of care in collaboration with patients, families, caregivers, and the health care team
• Communicate with medical teams and patients regarding post-discharge care needs, including home health and inpatient rehab
• Coordinate with post-acute care referral sources and place orders for home health, DME, and nutritional needs in Epic
• Candidates must reside more than 75 miles from the facility to be eligible for this position
