American Traveler is seeking an experienced RN Case Manager for a Home Health position in Vermont and New Hampshire, requiring a minimum of 2 years of home health experience and an active VT or compact RN license.

Details

• Home health setting covering more than 70 towns across Vermont and New Hampshire
• Service areas include Bradford, Randolph, Springfield, and White River Junction, VT as well as Lebanon, NH
• Case management role involving nursing, rehabilitation, hospice, and personal care services
• Day shift schedule
• 13-week contract

Requirements

• Active RN license in Vermont and New Hampshire, or an unencumbered compact license
• Current BLS certification required
• Minimum 2 years of home health experience required
• Graduate of an accredited school of nursing

Additional Information

• Develop and periodically review individualized care plans in collaboration with the patient's physician, patient, and family or primary caregivers
• Coordinate patient care through interdisciplinary communication and case conferences, advising other disciplines of discharges in a timely manner
• Conduct ongoing assessments of patients' clinical, functional, and emotional status, communicating significant changes to the physician and colleagues
• Educate patients and caregivers on self-care techniques, medications, diet, and health counseling as ordered by the physician
• Document evaluations, treatment goals, and objectives
• Work with broad independence while remaining an integral part of a cohesive interdisciplinary care team
