American Traveler is seeking an experienced Home Health RN for a 13-week travel contract requiring a CA RN license, home health experience, and availability for weekday and weekend shifts.

• Home health care setting with visits made to patients in their homes
• Patient population consists primarily of recently discharged, high-risk-for-rehospitalization adults — no pediatric patients
• Common diagnoses include CHF, COPD, wounds, wound vacs, and IV/PICC line management
• Typical daily caseload is approximately 5 visits per day, based on a point system (SOC: 2.5 pts, resumption of care: 1.75 pts, recert: 1.5 pts, routine visit: 1 pt) with a target of 5.25 points per day
• Charting is done in Cerner
• 8-hour day shifts, 5 days per week
• Schedule runs either Monday–Friday or Tuesday–Saturday (Tuesday–Saturday preferred)
• Every other weekend required
• No on-call or standby requirement
• Floating within home health only
• 13-week contract

Requirements

• Active CA RN license (or Compact license valid for California) — must be active at time of consideration; pending licenses not accepted
• Current BLS certification required — pending certifications not accepted
• Valid driver's license and current auto insurance required
• Minimum 2 years of RN experience required
• Home health experience strongly preferred
• Experience with HomeCare HomeBase (HCHB) preferred
• Cerner EMR experience preferred
• 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire
• Copy of driver's license required for consideration

Additional Information

• Conduct home visits including start of care, resumption of care, recertification, and routine visits
• Mileage reimbursement provided per GSA guidelines
• Supervisor is available during working hours for routine questions and after hours for urgent needs; an administrator on call is also available after hours and on weekends
• Home health aide support is available within the department
• Schedule is generally set by the previous day but is subject to morning changes due to unprocessed SOCs or staffing needs
• Holiday coverage may be needed for Thanksgiving and/or Christmas
• Any work-appropriate scrubs are acceptable
• Candidates may not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity facility within the past year; candidates placed PRN through outside staffing firms will be considered
• RNs must pass the Relias Dysrhythmia exam (basic or advanced) before an offer can be confirmed
• A pass/fail sterile technique test for Foley catheter insertion and central line dressing changes is administered on the first day of orientation
• 12-hour orientation
