American Traveler is hiring a travel RN Hospice Liaison for a community-based hospice role in northern Wisconsin, requiring at least 2 years of hospice experience and Epic proficiency.

Details

• Community-based, field-facing role serving patients in the Green Bay, Oshkosh, Manitowoc, Sheboygan, and Grafton areas of northern Wisconsin
• Role involves monitoring referral queues and consulting with patients referred from hospitals and SNFs for hospice evaluation
• Average caseload of 2–3 patients per day
• Adult patient population with diagnoses including heart failure, cancer, and other end-of-life disease processes
• May rotate across multiple coverage areas day to day (e.g., Green Bay/Oshkosh one day, Manitowoc/Sheboygan another)
• Assigned to 1–2 hospital sites depending on size and census
• EMR: Epic (Hyperspace and Epic Remote Client) — experience required
• Monday–Friday schedule with a flexible start time between 8:00–8:30am
• 40-hour work week, day shift
• No weekend rotation, on-call, or holiday requirements

Requirements

• Active WI RN license or valid compact RN license currently in hand
• Current BLS certification required
• Valid driver's license, personal vehicle, and active auto insurance required
• Minimum 2 years of hospice experience; hospice liaison experience preferred, but other hospice experience will be considered
• Epic EMR experience required
• Proficiency in end-of-life symptom management including pain and dyspnea
• Comfort with end-of-life medications and educating providers on their use
• Strong communication and collaboration skills with physicians and multidisciplinary teams
• Ability to educate patients and families on hospice benefits, philosophy, and levels of care (general inpatient, routine, respite)
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the past year
• Candidate DOB and last 4 digits of SSN required for consideration
• Middle name or initial required for consideration

Additional Information

• Conduct assessments and provide recommendations on appropriate hospice settings for patients transitioning from hospital or SNF care
• Admit patients to hospice services in the hospital setting when appropriate
• Laptop and phone provided
• Supervisor and manager available by phone and chat; liaison team and Hospice Medical Directors available for support
• Orientation includes new employee orientation (NEO) and 40 hours of unit-level orientation with a staff member
• Business casual attire or professional scrubs required; white coat preferred but not required
