American Traveler is seeking an experienced RN for an outpatient Hospice/Palliative Care position requiring a minimum of 3 years of home care or hospice experience and a valid NC or compact RN license.

• Outpatient hospice setting with both field (home) and hospital-based responsibilities
• Serves a hospice patient population requiring initial admissions assessments and end-of-life care
• Handles 2–3 admissions per day on a 10-hour schedule
• Schedule: 4x10-hour day shifts, 08:00–18:30
• No weekend or on-call requirements
• Floating required between hospital and field locations
• EMR: Epic (Dorothy/Comfort experience highly preferred); SBAR documentation framework used

Requirements

• Active RN license for NC (Standard or Compact) required; may be submitted with license pending
• Current BLS certification required
• Minimum 3 years of experience in home care or hospice required
• Home health/hospice and skilled nursing facility experience required
• Proficiency in intake assessment/interviewing, patient and family education, and DME/medication/supply setup for new patients
• Strong written, organizational, and analytical skills required
• Must be able to travel between multiple locations in varying weather and road conditions
• Two professional references from a manager or supervisor within the past 2 years required (pending at time of consideration is acceptable)

Additional Information

• Conducts comprehensive initial clinical assessments including head-to-toe evaluation, medical history review, and clinical summary presentation to the Hospice Medical Director
• Initiates the plan of care and coordinates orders with the Hospice Attending provider based on assessed patient needs
• Consults with physicians to obtain appropriate orders and arranges necessary supplies, equipment, and medications for new patients
• Evaluates hospitalized patients against regulatory criteria and assists with discharge planning
• Completes clear, concise, and timely clinical documentation related to each patient's initial hospice admission and assessment
• Inpatient hospice experience and a strong understanding of hospice admission LCD criteria are preferred
• Cultural awareness and end-of-life care sensitivity are preferred
• Orientation is approximately 2–3 weeks (roughly 80 hours); candidates are expected to arrive with foundational hospice experience
• Pre-start modules (4–5 hours for non-acute roles) must be completed before the assignment begins; modules are completed via online learning, Zoom, webinar, or in-person classes
• Navy scrubs required; not provided by the facility
• Candidates must be available for a manager interview prior to offer
• No RTO allowed during the first week of the assignment due to orientation; up to 7 days of RTO may be approved if it does not fall on a holiday
• Candidates must be available to work 2 out of 3 major holidays
• First-time travelers are welcome to apply
• Previous permanent, PRN, or EP employees of Novant Health or New Hanover must wait 12 months before applying as a traveler; facility rehire approval is required
• Local candidates are accepted; candidates residing within 50 miles of the facility may be eligible at a different rate
• Travel pairs are allowed, provided each individual has their own transportation and is comfortable with potentially different schedules
