Description

American Traveler is seeking an experienced RN Case Manager for a 13-week home care assignment in a post-acute setting, requiring a Minnesota RN license and at least 2 years of case management experience.

Details

• Post-acute home care setting
• Case Management unit
• 8-hour day shifts, Monday through Friday
• Point Click Care (PCC) is the EMR used in this position

Requirements

• Active Minnesota (MN) RN license required — must be active and in hand at time of consideration; pending licensure not accepted
• Current BLS certification required; copies of active certifications required — pending certifications not accepted
• Minimum 2 years of RN case management experience required; no exceptions
• Driver's license required for consideration
• 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both must include dates of employment and eligibility for rehire

Additional Information

• Candidates must reside more than 80 miles from the facility to qualify for travel pay rates; local candidates residing within 80 miles are accepted at a different rate
