American Traveler is hiring an experienced Hospice RN for a home health and hospice agency in Leander, TX, requiring HCHB experience and availability for on-call rotation and patient admissions.

Details

• Home health and hospice setting serving patients across a broad central Texas service area
• Caseload of approximately 4–6 patients per day, with a target of 30 points per week (equivalent to 40 hrs/week)
• Patient population includes geriatric and in-home hospice patients requiring skilled nursing care
• Cases include in-home patient care, interdisciplinary care coordination, and plan of care development
• Charting performed using HCHB (Homecare Homebase) and OASIS; charting must be completed at the patient's home
• Day shift schedule, Monday–Friday, 8:00 AM–5:00 PM
• Weekends required as needed
• On-call rotation required, including approximately one night of call per week (details discussed at interview)
• Travel up to 50 miles outside of home base required

Requirements

• Active TX RN license or compact RN license valid in Texas — must be in hand at time of consideration, no pending licenses accepted
• Current BLS certification required
• Minimum 1 year of hospice experience required; minimum 2 years of overall clinical experience required
• Experience with HCHB and OASIS required; 1 year of experience with both is preferred
• Must be prepared to perform patient admissions
• Must be a non-smoker due to patient population sensitivity (COPD patients)
• 2 professional references from a supervisor within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration

Additional Information

• Provides skilled nursing care to hospice patients in their homes, coordinating care with families, certifying providers, and the full interdisciplinary team
• Responsible for comprehensive patient assessments and collaborating with the certifying provider to develop and oversee individualized plans of care
• Must be assigned a tablet and prepared to see patients same-day
• Scrubs in any color are the required dress code
• Mileage reimbursed at the IRS standard rate
• Local candidates within a 50-mile radius are accepted
