American Traveler is seeking an experienced RN Case Manager for an adult outpatient PACE (Program of All-Inclusive Care for the Elderly) program, requiring 2 years of experience and Epic proficiency.

Details

• Adult outpatient care program setting focused on keeping elderly participants healthy and living independently in the community
• Case management unit serving a geriatric population, including participants recently discharged from acute care and skilled nursing facilities
• Position is primarily on-site with occasional community-based home visits to participant residences as needed
• Day shift schedule: 5x8-hour shifts, 8:00 AM – 4:30 PM
• No weekend or on-call requirements
• Floating between all facilities within the state may be required
• Charting via Epic (required)

Requirements

• Active WA RN license or compact RN license currently in hand (pending WA license accepted)
• Minimum 2 years of RN case management experience
• Epic EMR experience required
• Experience with geriatric populations highly preferred
• Ability to drive to participant residences in the community for assessments as needed
• At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting required for consideration
• SSN and DOB required for consideration

Additional Information

• Responsibilities include patient assessment, care planning, implementation, coordination, monitoring, and ongoing evaluation to ensure medically and financially appropriate care
• Works closely with the interdisciplinary team (IDT), Social Workers, and Physicians to coordinate cohesive, participant-centered care decisions
• Mileage is reimbursed for community visits; a laptop and cell phone are provided for use during the assignment
• Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the facility
