American Traveler is hiring a travel RN Hospice Case Manager serving Waukesha and Jefferson County with a minimum of 2 years of hospice experience and Epic Remote Client familiarity preferred.

Details

• Community-based hospice setting servicing Waukesha and Jefferson Counties
• Exclusively serves adult end-of-life patients
• 1:1 patient ratio
• Uses Epic Remote Client EMR (experience preferred)
• Uses Sapphire IV pumps
• Monday–Friday schedule, 8-hour day shifts from 0800–1630; four 10-hour shifts may also be available
• No weekend, on-call, or holiday requirements
• May be required to float anywhere within the serviced counties

Requirements

• Active WI or Compact RN license required
• Current BLS certification required
• Valid driver's license and auto insurance required
• Minimum 2 years of hospice nursing experience required
• Experience with Epic Remote Client preferred
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the last year
• Middle name or initial, DOB, and last 4 digits of SSN required for consideration

Additional Information

• Responsibilities include end-of-life care and planning, wound assessments, care coordination, pain and symptom management, and triage
• Supported by a team of RNs, a Resource Nurse, and Case Managers
• Orientation is up to one week based on individual comfort level
• Scrubs in any color or business casual attire are acceptable
• First-time travelers are welcome to apply if they have hospice and Epic Remote Client experience
• Local candidates are accepted
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
• A clinical interview with the clinical manager is required as a second interview step
