American Traveler is hiring a travel RN for a Home Hospice Case Manager position in the Chicago Southeast Suburbs, requiring at least 3 years of home hospice or home health experience and a valid IL RN license.

Details

• Home hospice setting — all patient care is provided in patients' homes across a defined service territory including Lynwood, Calumet, Calumet City, Calumet Park, Riverdale, and Dolton
• Adult patient population with diagnoses including failure to thrive, advanced cancers, end-stage COPD, end-stage disease processes, and end-of-life/terminal illness
• Caseload of 12–15 patients
• Monday–Friday, 8-hour day shifts (0800–1630), 40 hours per week
• No weekend rotation, on-call, or holiday requirements
• EMR: Epic Remote Client
• Occasional in-person team meetings and an initial on-site visit to the regional office in Oak Brook for supply pickup

Requirements

• Active IL RN license required
• Current BLS certification required
• Valid driver's license, personal vehicle, and active auto insurance required
• Minimum 3 years of home hospice or home health RN experience required
• Most recent experience must be within the last year
• Proficiency in wound care, dressing changes, wound VACs, drain management (Pleurex, nephrostomy, foley), and pain/symptom management
• Strong organizational and time management skills required
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN collected within the last year required for consideration
• Candidate DOB and last 4 digits of SSN required for consideration

Additional Information

• Responsibilities include care coordination, end-of-life planning, patient/family education, triage, and weekly wound photography and measurement
• Phone and laptop provided, along with trunk stock supplies upon orientation
• Support from fellow hospice RNs and management available by phone
• Orientation is 1–2 weeks, tailored to experience level
• Navy blue or solid professional-colored scrubs required — no graphics, logos, or designs
