American Traveler is hiring an experienced RN Case Manager for a Home Health position in the Milwaukee South area, requiring at least one year of home health experience and BLS certification.

• Home health field position serving patients in community settings including private homes, family member homes, assisted living facilities, group homes, and nursing homes across select Milwaukee-area zip codes
• High-acuity patient population involving pleurx drains, PCA pumps, IV antibiotics, TPN, Foleys, wounds, wound VACs, chest tubes, and various drains
• RN Case Manager caseload is 22–25 patients per week with approximately 6 visits per day
• EMR: Epic (experience preferred)
• Monday–Friday schedule, 8-hour day shifts from 0800–1700, with availability required by phone during those hours
• Daily huddle at 0750, which can be attended by phone or Teams if en route to patients
• Flexible schedule options (M–F or Tues–Sat); candidates open to occasional Saturday work are preferred
• No weekend rotation, no on-call, and no holiday coverage required
• May float to neighboring areas based on census

Requirements

• Active WI RN license or compact RN license currently in hand
• Current BLS certification required
• Minimum 12 months of home health RN experience, with most recent experience within the last year
• Proficiency in patient and family education, provider communication, and disease management
• Knowledge of and ability to troubleshoot pleurx drains, nephrostomy tubes, trachs, PCA pumps, and gastric suction devices (performance of these tasks is not required)
• One professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN
• Candidate DOB and last 4 digits of SSN required for consideration
• Candidate middle name or initial required for consideration
• Must be N95 fit tested prior to start

Additional Information

• Orientation is up to 2 weeks based on individual comfort level; on-site training provided for equipment troubleshooting as needed
• Support team includes fellow Case Managers, providers, a Clinical Manager, Chaplain, and Social Workers
• Laptop, cell phone, and PPE are provided
• Any scrub color is acceptable
• Local candidates are accepted at the same rate as travelers; no radius requirement
• First-time travelers are welcome to apply, provided they have at least one year of home health experience
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible for this position
