American Traveler is seeking an experienced RN Case Manager for an acute inpatient case management position requiring a CA RN license and a minimum of 2 years of experience.

Details

• Acute inpatient hospital setting in a Case Management unit
• Focuses on care coordination and care transitions throughout the acute care patient experience
• Collaborates with physicians, utilization managers, medical social workers, bedside RNs, interdisciplinary teams, third-party payers, and community resources
• Responsibilities span utilization management, care coordination, and care transitions planning
• Day shift, 5x8-hour shifts (8:00–16:30)
• Every other weekend (EOW) required

Requirements

• Active CA RN license required
• Current BLS certification required
• Minimum 2 years of RN experience required
• Acute inpatient case management experience required
• Candidates must reside more than 50 miles from the facility to qualify for travel rates
• Two supervisor references obtained within the past year required for consideration

Additional Information

• Responsible for reviewing admission care plans and gathering medical, psychosocial, and financial information to assess readmission risk and coordinate appropriate care
• Monitors length of stay (LOS), level of care (LOC) requirements, and facilitates timely transitions to appropriate post-acute settings
• Maintains current knowledge of CMS and Joint Commission Transitions of Care requirements, Conditions of Participation (COPs), and other regulatory standards
• Documents consistently in the EHR and other electronic software platforms
• Identifies and addresses complex clinical and social situations, including social determinants of health, ED high utilizers, and 30-day readmissions
• Candidates who have previously worked at this facility must have been away for at least 1 year before returning as a traveler
• Only one holiday RTO request will be approved for the duration of the contract
