American Traveler is seeking an experienced RN Case Manager for a complex case management position in the DC area, requiring a DC RN license and a minimum of 2 years of acute or post-acute care management experience.

Details

• Full-time day shift schedule: 5x8-hour shifts, 8:00 AM–4:30 PM
• 40 hours per week with no on-call or weekend requirements
• Manages a chronic and high-risk patient population including High Risk Pregnancy, Postpartum, DM, CHF, COPD, Asthma, obesity/overweight, and all Behavioral Health diagnoses
• Case managers conduct outreach calls to enrollees in the Washington, DC area to complete comprehensive assessments, develop care plans, and manage ongoing care
• Field visits may be required up to 2 days per week, with mileage reimbursement provided
• EMR used is Guiding Care/Healthedge
• Position is open to local candidates in the DC area only

Requirements

• Active DC RN license required; pending DC RN license accepted, but must be in hand prior to start date — proof of pending status required for consideration
• Current BLS certification required
• Minimum 2 years of case management experience required, including Acute Hospital, Long Term Acute Care/Rehab/Skilled Nursing, Admission Criteria, Care Coordination, Discharge Planning, InterQual Criteria, and Milliman Guidelines
• Previous charge nurse experience preferred
• NCQA experience preferred
• Regulatory knowledge required: CPT codes, Department of Health DRGs, HEDIS measures, NCQA, OSHA procedures, and JCC
• Must reside within 60 miles of the facility to qualify for this position

• Responsibilities include conducting comprehensive assessments, developing individualized care plans, and providing ongoing case management within the EMR
