American Traveler is hiring an LPN for a home care position requiring home care experience and delivering direct patient visits under RN and Clinical Team Manager direction.

Details

• Home care setting through a VNA nursing program
• Works within an interdisciplinary team including RNs, PTs, OTs, and HHAs
• Provides approximately 6 patient visits per day
• Schedule is Monday through Friday, 8:00 AM to 4:30 PM
• Requires a weekend rotation of 1 in 4 weekends, equating to one weekend shift per month
• Uses Epic for charting

Requirements

• Current LPN license required if applicable, per state requirements
• BLS certification required (AHA preferred; Red Cross, CHSF, or Military accepted at some locations, though certain sites require AHA only)
• Home care experience required
• Primary source verification of professional license and/or certification required within 30 days of start and updated upon expiration
• Both NURSYS and Board of Nursing verification required for nursing licenses
• Conditional or pending licenses not accepted
• Flu vaccination required with no religious or medical exemptions allowed

Additional Information

• Deliver skilled nursing care to patients as delegated by the RN Case Manager and/or Clinical Team Manager
• Perform nursing functions in accordance with the State Nurse Practice Act and established organizational policies
• Local candidates are accepted; permanent residence must be within 60 miles of Boston, MA for standard local rate eligibility
