American Traveler is hiring a travel RN for a Hospice and Palliative Care Home Health position requiring a minimum of 2 years of home hospice RN experience and a valid NJ driver's license.

Details

• Home health/hospice setting with community-based patient visits
• Unit: Homecare, Hospice, and Palliative Care
• PM shift, 11:00 AM – 7:00 PM, 5 days per week, 37.5 hours per week
• Every other weekend required
• On-call responsibilities required
• Floating to a nearby sister facility may be required
• EMR: Epic (experience required)
• 13-week contract

Requirements

• Active NJ RN license (NJ or Compact) required
• Current BLS certification required
• Minimum 2 years of RN experience in a home hospice environment required
• Prior General Inpatient (GIP) hospice experience within a hospital setting required
• ASN or Diploma with NJ RN license required (BSN preferred)
• Valid New Jersey driver's license required
• Strong critical thinking and communication skills required
• 2 professional references required for consideration

Additional Information

• Responsibilities include conducting patient assessments to evaluate appropriateness for hospice care, developing and revising individualized plans of care for patients and families
• Manages a caseload with full case management responsibilities, including prioritizing care needs, reconciling medications, and ensuring appropriate service utilization
• Documents assessments, re-visits, and care activities using a laptop, with data transfers required twice daily
• Participates in interdisciplinary team (IDT) meetings every two weeks and collaborates with the full care team to communicate patient progress
• Provides patient and family education using age-appropriate teaching methods, evaluating and revising teaching plans as needed
• Attends monthly staff meetings and participates in performance improvement (PI) and utilization review (UR) activities as requested
• Candidates must reside more than 50 miles from the facility to be eligible; local candidates are not accepted
• Travelers may remain on assignment for up to one calendar year from the initial start date; a break is required before returning
