American Traveler is hiring a compassionate Hospice RN for a home-based end-of-life care role in Moab, UT, requiring a minimum of 3 years of nursing experience and an active UT or compact RN license.

Details

• Home-based hospice care setting, with care delivered in patients' preferred place of residence
• Interdisciplinary team environment including physicians, social workers, chaplains, and volunteers
• Day shift schedule
• On-call rotation required 1–3 shifts per week, including 3 weekends over the course of the 13-week contract (dependent on staffing levels)
• Must be able to respond to patient needs within 45 minutes when on call
• Valid driver's license and reliable transportation required for home visits
• Hospicesoft/Careficient EMR experience preferred
• Holiday coverage required (New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Day)

Requirements

• Active RN license in Utah or a compact state currently in hand
• Minimum 3 years of nursing experience, preferably in hospice, home health, or palliative care
• Current BLS certification required
• Valid driver's license and reliable personal transportation
• Strong clinical assessment skills
• Pre-employment drug test and background check required

Additional Information

• Perform comprehensive patient assessments and coordinate individualized end-of-life care plans
• Provide pain and symptom management in accordance with the hospice plan of care
• Educate and support patients and families on disease progression, self-care, and hospice philosophy
• Maintain accurate and timely documentation in compliance with state, federal, and organizational requirements
• Blue scrubs preferred
• Located in Moab, UT — a vibrant outdoor recreation hub surrounded by Arches and Canyonlands National Parks, the Colorado River, and the LaSal Mountains
