American Traveler is seeking an experienced RN for a Float Home Health position in the North Region of the Greater Boston area, requiring at least 1 year of certified home care and case management experience.

Details

• Home health setting serving patients across multiple Boston-area neighborhoods including Back Bay, Beacon Hill, Cambridge, Charlestown, Fenway, South Boston, South End, and surrounding communities
• Float role requiring travel to patient cases as assigned throughout the North Region
• Day shift schedule: 8:00 AM – 4:30 PM
• Weekend rotation required once every 6 weeks
• 13-week contract
• Epic used for electronic charting
• UKG/Kronos used for timekeeping

Requirements

• Active MA RN license required — no pending or conditional licenses accepted
• Minimum 1 year of certified home health experience required
• Minimum 1 year of case management experience required
• All certifications must be issued by the American Heart Association (AHA)
• Valid driver's license and ability to travel independently to patient cases as assigned
• 2 professional references required for consideration
• Flu vaccine required with no religious or medical exemptions accepted
• Primary source verification of professional license required within 30 days of start date; both NURSYS and BON verification required for nursing licenses

Additional Information

• Responsibilities include assessing patient conditions, developing and implementing individualized care plans, and providing education and support to patients and families
• Performs admission processes and case management duties in accordance with organizational policies and standards of community health practice
• Utilizes thorough and timely electronic documentation and coordinates interdisciplinary communications
• Patient care is delivered in alignment with organizational philosophy and community health practice standards
• No validated parking is available at Boston locations; public transportation or paid public parking lots are available (approximately $17–$22/day); parking costs may apply at other locations as well
• Local candidates residing within 60 miles of the Boston facility are eligible to apply
