American Traveler is hiring a travel RN - Case Management for a 13-week acute inpatient care coordination role requiring an active VA or compact RN license and at least 1 year of acute care experience.

Details

• Acute inpatient hospital setting focused on patient care coordination and case management
• Responsible for coordinating care across the continuum for admitted members, including discharge planning and post-acute transitions
• Works with a multidisciplinary team including physicians, nursing, utilization review, and community/ambulatory providers
• Coordinates post-acute services including Home Health, DME, IV Infusion, SNF Sub-Acute, and Acute Rehab
• Shift: 8:30 AM – 5:00 PM (8-hour days), 40 hours per week
• Every other weekend required
• Uses Epic EMR

Requirements

• Active VA or compact RN license required; must be in hand at time of consideration — pending licensure not accepted
• Associate's Degree in Nursing (ADN) required
• Minimum 1 year of experience in an acute medical/surgical, ED, or critical care nursing setting
• Knowledge of health care reimbursement systems including Medicare Advantage, Commercial payer, Medicaid, and CMS regulatory rules
• Experience using an electronic medical record (EMR) system
• Last 4 digits of SSN and date of birth required for consideration

Additional Information

• Conducts initial face-to-face assessments for every admitted patient within 24 hours of admission to identify discharge needs
• Documents and communicates assessment outcomes and discharge plans in accordance with facility and regulatory requirements
• Sends referrals and coordinates with in-network vendors for post-acute levels of care
